Pediatric Ophthalmology and Adult Strabismus offers online patient forms you can complete from home or the office. If this is your first visit, please download and complete the forms below. You can email or fax them to us or bring them to your appointment. Completing the forms in advance will help reduce your check-in time. All forms are interactive and can be filled out directly on your computer. 

To transfer medical records from a previous physician to us, please complete the appropriate records release form. You can also complete a form to transfer records from us to another physician.
